(Batesville, Ark) – Gary Bebow, Administrator/CEO of White River Health System (WRHS), is pleased to announce the promotion of Jennifer Sandage, RN, to Director of Case Management at White River Medical Center (WRMC).
As the Director of Case Management, Sandage’s responsibilities include overseeing the daily operations and outcomes of the Case Management Department. She supervises Case Managers, RNs and Social Workers. Case Managers ensure patients are admitted and transitioned to the appropriate level of care. They ensure patients have an effective plan of care, receive prescribed treatments, and have an advocate for services and plans needed during their stay. Case Managers undertake assessment, monitoring, planning, advocacy, and linking patients and families with post-acute care services and other support services.
The goal of Case Management is to improve patient outcomes, reduce readmission risks, and eliminate system delays or inefficiencies in the delivery of care. Case Management ensures services provided are medically necessary, whereby enhancing the hospital’s ability to collect appropriate revenue for services provided.
